It may have been bookmarked. Does the location bar show a yellow/gold star for that site, if it does it means the site has been bookmarked. You will need to delete the bookmark or stop the location bar displaying bookmarks. You can do that bu changing the setting on the bottom of the Settings for privacy, browsing history and do-not-track to only display history, or change it to display nothing.
It's not bookmarked. It has a page symbol beside it- no star.
Try forcing Firefox to forget about the site.
- Open the history sidebar - use keyboard shortcut Control+H, or in the menus select View > Sidebar > History
- Find the site in the history, then right-click on it and choose "Forget about this site"
That should remove all traces of the site from history/bookmarks/cookies/passwords/form data.
I cleared out all of my history so it's not showing up in the side bar under history.
If you highlight the site in the location bar and press the Delete key, does it get deleted?
No. I tried that.

You can try "Forget about this site" in the right-click context menu in the history side bar (Ctrl+H) after opening that site to get a fresh history item. You can click the View button and sort by last visited.

The file places.sqlite stores the history and the bookmarks in Firefox 3 versions, so there may be a problem with that file if you can't remove some history or bookmarks items.
