During the install did you get a option to create icon or create a shortcut option?

No, I did not. I retrieved the FF download from here: https://www.mozilla.org/en-US/firefox/new/

The installer file is version 4.42.

I thought I might have missed it, so I re-installed; but, there was no chance to customize the install.

I tried to attach screenshots, but this page is taking unacceptably long to upload a small png file - 111kb.

Hi, try this please : uninstall Firefox. Then Delete the Mozilla Firefox Folders in C:\Program Files , C:\Program Files(x86) & C:\ProgramData Then restart system. Then run Windows Disk Cleanup. (Note: This should be Pinned and run Weekly, If never done below expect 10's of gig's(Win10)) Then run it again and click the button that says Cleanup System Files. Note: your Firefox Profile is saved. But you should make a back up before you do :

Reinstall with Current Release Firefox 59.0 with a Full Version Installer

This did not solve my issue. I did not have FF on my system prior to the 1st install a week or so ago.

I did a clean re-install of Windows and just installed FF again this morning that had the same result.

The issue is with the installer. You can verify the behavior by trying it yourself, which will take less than 2 minutes.
