Two weeks ago I reloaded Windows 10 because of a suspected malware infection. This required me to reload my apps. After reloading Firefox the custom icons have disappeared from some, but not all of the toolbar menu items. Why? Rather, a little globe appears, which I'm guessing is a default icon. -- Why did some of the icons disappear while others did not? -- How do I remove the "little globes" as they're just taking up space and have no meaning -- Where do the custom items come from?

I assume that you are talking about the bookmarks on the Bookmarks Toolbar that no longer have the website's favicon, but have the default globe like icon.

How did you restore the bookmarks ?

Using Sync or restoring a JSON backup doesn't include the favicons and you would have to click a bookmark with the default globe icon to make Firefox retrieve the favicon from the server.
