Buttons such as reply all no longer work unless I disable the lightning add-on
In the (significant) time I have spent trying to diagnose this problem, both TB 38.7.0 and Lightning v 4.0.7 were released. I installed both updates, but I still cannot use Reply All or Send (nothing happens) or save a draft email unless I disable Lightning. One issue which seems to be resolved by this update is that drop down boxes have started working (e.g., changing address from To: to CC:). I am running Win 7 Pro SP1, 64 bit build 7601 on a Lenovo W520.

Remove the Lightning add-on. Restart Thunderbird and re add the add-on.
Does that clear it?
I removed the Lightning add-on, rebooted and restarted TB 38.7.0, and still could not reply to messages. I did have 3 other extensions active, DoD Config 1.3.6, LookOut 1.2.13, and Signature Switch 1.6.14. I then disabled each of these and restarted TB, and appeared to regain full email functionality. I'm not sure which of these 3 is responsible for the conflict.
I then tried to reinstall Lightning 4.0.5.2, and was told it is not compatible with my version of TB. At https://developer.mozilla.org/en-US/docs/Mozilla/Calendar/Calendar_Versions, v 4.0.7 is listed as compatible with TB 38.7.0, but there is no link to this version of Lightning. I would roll back my version of TB, but have been unable to find any link to previous versions. So, right now I do have (I think) a working TB client, but no Lightning calendar or other extensions.

It is an interesting game we play, 1.03 and 4.07 will be identical versions. The higher number is distributed with Thunderbird while the loger site on AMO for download. It is done that way to prevent install with the application then immediately an update. Sounds minor, but in a significant number of users machines it made Lightning mess up Thunderbird and symptoms like your having ensued.
To get the correct version for you of an add-on listed on add-ons mozilla.org. Simply go to the add-on manager and use the search box to search for lightning. Using this method the Thunderbird version is passed to the web site and you get offered the appropriate version for you, if there is no appropriate version, the install button is disabled.
If I had to guess, the signature switch would be my first coice as it interacts with the compose process.
