I click on the customize window and select the "icons" button and the bookmark toolbar stays on "icons and text". I select the "restore default set" button and the bookmark toolbar still stays on "icons and text" even though the default setting is supposed to be "icons" only. There is not enough space on this toolbar for both icons and texts when you accumulate a lot of bookmarks.

That setting only applies to the labels on toolbars and not to bookmarks on the Bookmarks Toolbar.

Add code to the userChrome.css file below the default @namespace line.

The customization files userChrome.css (user interface) and userContent.css (websites) are located in the chrome folder in the Firefox profile folder.
