Feedback banner appears each time Firefox 17.0.1 starts.
Whenever I start Firefox version 17.0.1 a banner appears asking if I want to provide feedback on the product. It does not matter if I say "yes" or "no" as the banner appears the nest time it starts. A second instance does not have this banner.

Please create a screenshot of the problem you're seeing by following the guide mentioned at How do I create a screenshot of my problem?.
Once you've done this, attach the saved screenshot file to your forum post by clicking the Browse... button below the Post your reply box. This will help us to visualize the problem.
As you requested here is the screen shot. The banner I cannot remove is the fourth one from the top. Thanks, Don
That is this setting:
- Tools > Options > Advanced > General: System Defaults: "Submit performance data"
You should have and keep these two prefs that you can inspect on the about:config page.
- toolkit.telemetry.prompted - value: 2 (Integer)
- toolkit.telemetry.rejected - value: true (Boolean)
