In my address book I had a column for work phone but it is gone since an upgrade was done. How do I get that column back?

I'm afraid that it's gone, unless some code is written to display it again. All phones numbers are bunched out in a column 'Phones'. If what you want is to search for a given phone number, you can still do it with advanced search, use Edit / Find / Search addresses...
The CardBook add-on allows for separate columns for all phone numbers.
