The easiest way to open a link in a new tab (and that includes bookmarks) is to middle-click the link (bookmark) with the mouse scroll button.

Bookmarks on Firefox's Bookmarks Menu or Bookmarks Toolbar? It's normal for them to open in the same tab you are currently viewing. You can use some of the same techniques you use on links to open a bookmark in a different tab or window:
- Hold the Ctrl key while clicking => new tab
- Hold the Shift key while clicking => new window
- right-click and choose from the menu
If you want to automate how bookmarks open so they always open in a new tab, there is at least one extension that offers that feature: Tab Mix Plus.
(Note: the attached image is from August 2012 and the dialogs might look a bit different now.)
Not sure about always opening them in a new window.
Also, if you right click, the menu will include; Open New Tab and Open New Window.
