How do I get rid of iComment that comes up when I open Firefox?

I'm not sure how it got into my computer. I have looked everywhere in order to try and delete the program, but to no avail. It's a window that opens up on top of my Firefox opening window and wants me to change to my name from their generic name (or what have you). There is also an iComment icon at the end of the navigation bar and I don't know how to get rid of it. I do not want to follow through and provide my name into whatever that want or do. Any thoughts as to how I can get rid of it?

Do you have that problem when running in the Firefox SafeMode?
