In order to customize printout header and footer it is possible to update the Firefox configuration page according to an answer in this forum. However, I do not know where to find this page on Ubuntu 18.04.

Hi tpollak, could you check the Page Setup dialog using either:
- menu button > Print (opens Preview) > Page Setup button
- File > Page Setup
