Neither Firefox nor Safari Can Schedule Taxis

I need to schedule a taxi appointment.

I am multiply disabled, and don't have accessible public transportation, and can't use phones, and can't use browsers without strobe-blocking fixes. All the local taxi companies, and most American taxi companies, use the same registration software on their websites. I can't get through this software in either Firefox or Safari, even after enabling pop-ups, enabling all cookies, and emptying the cache. I can't use safe mode because it isn't safe.

I also filed a report for tech evangelism:

And emailed the taxi company, and emailed my doctor, and cancelled the appointment I was trying to get to.

This site seems to have a problem processing dates. As shown in the attached screenshot, it reports a date of 12/1/2012 for any date I tried during January 2017. Firefox is sending the date I entered to the validation script, but there is some kind of problem on the back end, seemingly, which causes the current date/time to be returned to Firefox. ???
