Flash continually crashes on Macbook Pro after troubleshooting

Flash is up to date (v. 16) and FF is v.35 Even after unchecking Accelerate - and trying in Safe Mode, flash still crashes each time a page loads with a video or ad that uses flash.

I am on an older v of OS X 10.8.5
