Using Firefox for Android 44.0.2 I have no menu or reload buttons available within the browser.

Running Android 5.1.1 on OxygenOS (OnePlusX) - so my hardware buttons are assigned for other tasks on long press.

The solution should arrive in Firefox 45 for Android, currently available in the Play store as Firefox Beta, in the form of a menu button on all devices:

I don't know how to transfer settings from the non-beta version to the beta version, but hopefully someone else know the best way. Or if you don't really have any settings to transfer, you could just give it a try.
