Since I installed Firefox 38 for Mac, the "traffic lights" don't show in popups and pop unders. Today I installed Firefox 38.0.1 and the problem remains.

I found the culprit: it was the Simple White theme (version 2.2b16). I removed it and now everything seems to be working perfectly.
