My exceptions list for sites allowed to load images, (permissions.sqlite in the profile folder I think) is cleared on exiting Firefox, and/or when I clear recent history. These images stay blocked for the session. I never allow exceptions for cookies or pop-ups. I have cleared the box for site preferences, and the other 2 boxes in the data area of the history settings menu, but the list is still cleared on exit. I've never had this problem until about a week ago, and have been running FF since version 0.8. This is FF 12, running on Win XP.

If you use cleanup software like CCleaner then check the settings for the Firefox application.
It is also possible that the permissions.sqlite is corrupted, so you can try to remove that file and let Firefox create a fresh copy to see if that helps.
- Do not use "Clear history when Firefox closes" to clear the "Browsing History".
- Do not use "Clear Recent History" to clear the "Cookies" and the "Site Preferences"
Clearing "Site Preferences" clears all exceptions for cookies, images, pop-up windows, software installation, and passwords.

- Make sure that you are using the "Remember History" setting as described in this article -> Settings for privacy, browsing history and do-not-track
- IF you use Delete browsing, search and download history on Firefox feature, DO NOT select Cookies, Site Preferences as described in this article -> Delete browsing, search and download history on Firefox
Check and tell if its working.

Problem solved by deleting the old permissions.sqlite file as suggested by cor-el. Thanks also to mha007 for the references to knowledge base articles concerning my problem. Very helpful.
